Monthly Maintenance Report.

This is a Federal Law, D.O.T. requirement, not merely a company policy. This report MUST be completed and submitted to our office no later than the 15th of the following month.

Driver info

This is a combination Repair Record, Lubrication and Grease Record, and Time Repair & Replacement Record. It must be completed each month in which maintenance is performed. List all repairs, lubrications, and/or replacements completed during the month. This also includes major items such as lights, reflectors, etc. All records must be listed for all power units. The Repair or Replacement Record lists all tire and brake repairs, replacements, etc.
